We just know because it's our job. They really look quite different to the trained and experienced eye. In case of doubt - less than 1% of the time - we can always take an x-ray to verify what's what.
I'm sure that you can do things on your job that would baffle me that come very easily to you. Identifying baby teeth is like that for a dentist.

Usually by the shape of the tooth and the position in the mouth in relation to the other teeth. They can also take an x-ray if needed.
